Skilled Nursing

•  Assess for changes on patients’ medical conditions,
• Educate on disease management and prevention of complication,
• Train health workers or patients in procedures such as skin care and tube feedings,
• Assist patients in managing the symptoms of chronic conditions and treating wounds,
• Perform procedures such as catheter insertion, ostomy care and insulin administration.

Physical Therapy

• Determine home equipment needs,
• Therapeutic exercises and balance activities,
• Restore balance, range of motion, strength and endurance,
• Ambulation training to regain functional mobility and motor skills,
• Reduce risk of falling and improve ability to walk, or to get up and down.

Occupational Therapy

• Recommend adaptations to make the home safer,
• Techniques for sensory integration and energy conservation,
• Establish a daily routine that allows patients to do important tasks without overtaxing,
• Help patients improve in daily tasks such as bathing, dressing, and basic household activities.

Speech Therapy

• Help with memory problems

• Restore ability to swallow safely,

• Treatment and management of voice disorders, aphasia, and dysphasia,

• Special techniques to improve and patient’s communication skills.

Medical Social Worker

• Assistance in locating care facilities,

• Counselling for long range planning and decision-making,

• Social and emotional counselling, community resource planning and referrals,

• Locate community resources to help with financial concerns.

Certified Home Health Aide

• Provides personal care and other services under the supervision of the licensed professional,
• Assist patients with personal hygiene and grooming,
• Help patients with bathing, dressing, toileting, feeding, and incontinence care,
• Assist with professionally established exercise and ambulatory programs,
• Report changes in patients’s condition to the skilled professional.
